So far, I've given only 10 titles 10/10 ratings. They are...

Berserk - The best part of the Berserk manga animated. What's not to love? It's a dark fantasy fans wet dream.

Code Geass (S1) - The only series that's come close to thrilling me like Death Note, mainly thanks to Sunrise stealing a fair amount from Death Note. It's just too bad about the second series...

Death Note - There's really just no thriller at the same level. If not for Madhouse messing up the ending (after doing a fantastic job of adapting the story) it'd probably be #1 in my rankings and not #2.

Gankutsuou - Gonzo retold the story of Edmond Dantes, changed a lot and still managed to make a fantastic anime. Very few people could do a good job of reworking a 1250 novel in order to fit it into a 24 episode TV series. While it isn't quite as good as the novel, it's still one of the best revenge stories I've seen. It's my #1 anime for good reason.

Grave of the Fireflies - Unlike with a lot of animated films that try to fit overly complex plots into 2 hours, there was nothing hard to understand about this realistic story of children starving to death during the war. For people with a heart, I'm sure it's a difficult film to watch because of how it pulls at the heart strings.

Gungrave - Considering it was based on a game with near to no story, I'd say Madhouse did an excellent job of creating what turned out to be the best mobster anime around. Michael Corleone has nothing on Harry MacDowel.

Macross Zero - A beautiful series in every respect. The clash of cultures made it absorbing, and the visuals were jaw dropping at times. Very underrated.

Now and Then, Here and There - Fed up of seeing stupid high school kids get transported to other worlds and have fun, usually going to a hot spring somewhere along the way? Then this is the anime for you. It's both shocking and wonderful. I can't tell you how happy I was to see a truly adult anime that involved brats getting transported away from Earth.

Princess Mononoke - GotF aside, the only Ghibli film I've scored above 8/10. It's been quite a bit since I watched it now... It's due a rewatch.

Sword of the Stranger - Best action flick ever. The fight at the end was truly awesome, it being fast-paced and never giving the feeling that it kept going just to make the film longer. Square, this is how you do sword fights - watch and learn.

There are some others that missed out by an inch or two (such as the first Kenshin OVA), and a few of the above are borderline 9/10, but I think my scores are pretty fair overall.


As for manga, I've yet to score anything 10/10. My current top five are...

Bitter Virgin - A wonderful story told maturely. However, the ending was pretty awful and let the series down.

Confidential Confessions - The series included many well written (and drawn) hard hitting, real life stories...but it also included some poor stories. I couldn't give something of a mixed bag 10/10.

Death Note - It takes too long to get going after the TS. As amazing as it is, the anime is more worthy of getting a 10/10 score.

Love Hina - If not for Vol. 11 and certain events towards the end making the story too far-fetched I'd have given it 10/10. It came very close.

Naruto - Something of a guilty pleasure and the 9/10 rating is only for Vol. 1-27. I can't give something with a poor second half 10/10.

Castle in the Sky - Best work Miyazaki has done, the rest of his works are 6/7/8.

Giant Robo - Watch it and you'll understand.

Ghost in the Shell: Innocence - My finger must have slipped. I dunno.

Grave of the Fireflies - Same as Giant Robo.

GunBuster - Bias, it's not that good, but everyone else here holds a little bias towards something.

DieBuster - It's not as good as GunBuster, so wtf? I still love it though.

EDIT: Forgot mango.

Berserk - It's Berserk.

One Piece - The BEST Shounen series ever.

Parasyte - I wait every three months for this to be released and it IS worth it.

Historie - Same manga-ka as Parasyte, this needs to be licensed even though it only has four volumes so far.

Was tempted to give GUNNM 10/10 but naw.

EDIT2: I really shouldn't do this... but Aion, 10/10 for Geass? Really now?
 
Yeah, first season of Geass is a big fat 10/10 for me, too.

There're two ways you can be a 10/10 series, for me - you can either be genuinely an all-time great series, clearly a fantastic work of art as well as being entertaining, or you can just be so entertaining that it feels churlish to focus on any small negative points. Geass falls into the latter category.

probably a couple that I'm being overly generous to, definitely a bit of a modernist slant, and one or two of the "bubbling under" that probably deserve to be in there, but that's close enough. Manga I'll do later, as that does require a bit more thought. ^^;

One of the greatest adventure films I've ever seen.

Whisper of the Heart
The touching story of an aspiring author's first love. The characters are so believeable that it's easy to forget that they're actually fictional.

My Neighbor Totoro
Unpretentious and joyfully nostalgic film that captures the wonder and innocence of childhood.

Kiki's Delivery Service
A spirited young witch leaves home, gets a job, and grows up. That's it. And it's amazing.

Ghost in the Shell: Stand Alone Complex 2nd GIG
The pinnacle of TV anime. Superior production values, direction, writing, plotting, and characterization. One of very few shows I would unreservedly recommend to adults who don't watch anime.

EPIC is not a big enough word.

Lupin III: The Castle of Cagliostro
THE greatest adventure film I've ever seen. Possibly the greatest film full stop. I don't think I'll ever tire of it.

The Girl Who Leapt Through Time
A wonderful, funny, charming film with one of the most likeable casts in memory.

The Wings of Honneamise
An ordinary man with an ordinary, directionless life overcomes his doubts and fears to achieve something incredible, to make history. An incredible feat of world-building.
 
The first season of Code Geass is a series you can ignore the flaws of and love. When you think about it, it was a bit silly at times, with giant robots skating around and all the rest, but the story was still very well written and the more epic segments more than made up for the school filler. The plot twists even made sense. And it had THE best cliffhanger ending ever.

I can't say the same about the second season. Full of repeated plot twists, fan service and illogical 'witsts', it failed hard in comparison to the first season. I dropped it after 8 episodes of pain.

A wonderful, funny, charming film with one of the most likeable casts in memory. The only negative is that the time travel side wasn't very well explained, leaving questions such as, "Why the hell is a stupid painting from the past so important?"

The Wings of Honneamise
An ordinary man with an ordinary, directionless life overcomes his doubts and fears to achieve something incredible, to make history. An incredible feat of world-building. And he also attempts to randomly rape a religious woman.

Anime that I rate 10/10 on MAL:

Serial Experiments Lain: I loved the complex storyline, the many hidden themes within it and the character herself.

The Girl Who Leapt Through Time: The ending was a bit disapointing imo, but it's the anime film that brought me back from my anime burnout. It's truly excellent imo with Makato being one of my favourite female protagonist.

Manga:

Bitter Virgin: I'm not a huge manga lover, but I loved Bitter Virgin a lot. The mature storyline, great execution of events and characters that will bound to have a few people who can relate to. I would like to see this get turned into an Anime or wishful thinking, an OVA.
 
Of course, no anime or manga is "perfect" so the 10/10 rating is a bit of a misnomer. I take it to mean that an anime is as brilliant as can reasonable be expected.

One of my all time favourite movies because of how gorgeous it is, how brilliantly scored it is and how entertaining and complex it is.

Fullmetal Alchemist
Great story, great characters, great music, great setting, and it's an incredibly exciting and immersive series.

Neon Genesis Evangelion
My all time favourite series for reasons I've enumerated many a time before on these forums! ;]

Ghost In The Shell (All)

Its ******* awesome. I'm so glad I picked this up. Even if the first gig was 40 ******* quid (I later found out it was 20 on Amazon, it was a moment where it was there and I was like '**** IT IL GET IT')


Death Note

Introduced me to Anime, and got me HOOKED big time. This show is just awesome.


Witch Hunter Robin

This show was so refreshing, and it reignited my love of Anime. I went through a long phase of watching nothing, then Witch Hunter Robin made me fall in love with Anime again. The beautiful art/animation, fantastic story, and awesome action sequences kept me watching everyday before and after school. (I got up at 0630 just to watch this)


Bleach

Best Shounen ever.
 
Anime

BECK - It has everything; Loveable characters who develop extremely well throughout the series, alot of the characters in the anime are in my favourite characters. Also, the designs and artwork for everything is.. awesome! The styles used on the characters, places, items, and everything is very eye appealing. As for the story, it's very addictive. The entire anime has some strange "fresh" feeling too it that us awesome. Even the soundtrack is awesome, too (it being a music-anime, n'all). Hoping for a season 2 of this anime.

Code Geass - Hangyaku no Lelouch - The plot is just.. incredible. From start the finish, I was pretty much gripped on this anime. The characters are all very unique and have stories behind them. The battles and struggles between the characters were very powerful, and you could feel Lelouch's passion and relate to the characters.

Toki wo Kakeru Shoujo - Well, don't know what to say. Basically I hadn't watched much anime in a while, then I watched this. It blew me away. The visuals were stunning, especially in HD. I liked the ending and also liked how the anime fitted into so many genre's. The anime really is suited for anyone; Different people view this anime and take different things from it.

Manga

BECK - Basically all the above, but more! The manga goes on three times longer than the anime so we see the characters and the story progress even more. The manga requires less patience and is alot faster moving than the anime. Although, it requires more faith because we aren't actually hearing the music; But, even then, it still gave me goosbumps at times, especially when Koyuki performs Devil's Way that was the best piece of anything I have ever seen, read, or heard.

Now, I've given Eyeshield 21 and One Piece 9/10's, but I'm still reading, I'd like to see further of the story before I give them 10/10. But, so far, they have both been about 9-10/10.

To be honest, I did this list mid-08, so I couldn't really remember much of X-Evolution except that it was Digimon, and it had sweet CG, so it got 10, until further notice, lol. As for the Naruto Special, that is just plain hilarious.

Berserk - Simply the best series I have ever seen, action and storyline were both awesome.

Devil Man - Without this I would have never started watching anime - I know a lot will disagree but I have so much fun watching this. Thks Channel 4

Castle In the Sky - Again another of the 1st ones I watched on Channel 4. I love the sense of adventure in this one

Akira - One that got me watching anime again after not really seeing much due to channel 4 not airing anything for ages and then my collection started :D

Cowboy Bebop (TV) - Need I say anymore

Freedom - Only one of recent that I have thought wow

Perfect Blue - Really surprised me this one

Vampire Hunter D: Bloodlust - Vampires and awesome animation, I got really caught up in this

Gungrave - I love a good gangster tale.
 
Manga:

Maison Ikkoku
Often imitated but never bettered. Takahashi's deft characterization, sense of comic timing, and ability to write incredibly intense scenes combine to make Maison Ikkoku a timeless classic.

Town of Evening Calm, Country of Cherry Blossoms
Two short stories about the effect of Hiroshima's atomic bomb on those who survived. One is set shortly after the blast, the other much later, yet they are connected. One is tragic, the other heart-warming, both are fantastic.

Solanin
Though I haven't finished this yet, it's already a certain 10. No other manga I've read has captured so perfectly the uncertainty around the transition from childhood to adulthood. The characters' humanity and humour mean it's practically impossible not to relate to them in some way.

I'm like a gaming magazine in that I would rarely give anything a full score - however those which have tipped over 96% and made me round up to a 10 are:

Jin Roh, Paprika, Millennium Actress, Perfect Blue, Ghost in The Shell (original film), Stand Alone Complex: First Gig, Cowboy Bebop Movie, Patlabor 2, Evangelion, Haibane Renmei, Kino's Journey, Yokohama Kaidashi Kikou, Paranoia Agent, Gunbuster, The Girl who Leapt Through Time, Only Yesterday, Grave of The Fireflies, Spirited Away.

Of those, the ones I feel really deserve a perfect score are:

Millennium Actress - My favourite Kon film. Plays with cinema conventions in a very pleasing way. A film depicting life as art, about memories and how people sometimes find it easier to fall in love with a romantic idea than can a person.

Perfect Blue - My second favourite Kon film. A brilliant psychological thriller with an aura of tensoin and paranioa only equalled by the same director's Paranoia Agent

Ghost in The Shell (original film) - My favourite Oshii film. Some people don't like the story. Some people don't like the end. Some people find it pretentious and boring. I absolutely love it. The marriage of Oshii's imagery with Kenji Kawai's haunting soundtrack ask better than anything I've read or watched: "What does it mean to be human?"

Patlabor 2 - My second favourite Oshii film (Copy in above for popular criticism) More of a political than a philosphical film but still ticks over in your brain for weeks afterwards. This film's message seems more relevant to us in the west now than it ever was. My first thought upon watching it was the tanks outside Heathrow Airport back in 2003. How far are we ever from martial law, and how easy would it be now for those who wished to enact it?

Haibane Renmei - Haibane wake up in a strage world, not knowing who or even what they are. The ones who do figure it out move on without being able to pass on the knowledge to others. Seems kinda familiar...

Only Yesterday - Another film about life and memories, albeit told in a more straightforward way than Millennium Actress. Really about growing up, changing dreams and priorites. Remembering the past but leaving it there and moving into the future.

I notice a few of yours are Ghibli films, which I do rate very highly (Hell, I own all the UK releases after all) but for the most part I can't bring myself to give them perfect scores. As a jaded 20-something aware of all the evils in the world I feel I'm missing out on the magical, captivating qualities they would have had on me as a child.
 
After around 500 volumes and boxsets of anime, I've given out a fair few 10 out of tens. Series that I reviewed volume by volume may have had the odd 10 out of 10 for a certain volume

Millennium Actress (Brilliant film)

I enjoyed the 1st Gig more than the second, and if I were reviewing boxsets, the first would get the 10, for a greater variety in stories, while the second would get a high nine

Of the two Ghibli films, my love for Kiki is undiminished, but Spirited Away has dropped a tad in my estimation in the years since I reviewed it.

Paranoia Agent V4 (It was a hell of a capper to a series, and if I were reviewing a boxset, it would get a 10)

Samurai Champloo V6, V7 (Not so for Champloo, which had an amazing conclusion, but a couple of episodes in the middle really sagged)

Heat Guy J boxset (Total guilty pleasure here. I love this show and I don't know why)

Full Metal Alchemist V11, V13 (I was harsh to FMA when I was reviewing it, because I didn't know how it would turn out. Taken as a whole now, it would be a 10 for me.)

The Wings of Honneamise (My second favourite movie of all time)

Beck V2 V3 (Would love to see how this story turns out)

Cowboy Bebop Remix Collection ('nuff said)

Kino's Journey V4 (Another one that grew on me as I watched it, a retrospective 10 for the series)

Black Lagoon V2
Black Lagoon: Second Barrage V1 V3 (Stunning popcorn series)

FLCL V3 (The release format annoyed me at first, then I realised this was true quality over quantity)

Mushi-shi V5 (Awesome series that I'm dying to see the conclusion of, I'm guessing this will be a retrospective 10 as well)


Melancholy Of Haruhi Suzumiya Collection (Yeah, I know, jumping on the bandwagon and all, but I loved this)

Planetes Collection (An entertaining story about realistic space travel. Who'dve thunk it? I know it's a Marmite anime, and I hate marmite. I love this though)

Azumanga Daioh Collection (I need me some Osaka)
The Girl Who Leapt Through Time (The best film release of 2008)

Welcome to the NHK Collection 1 & 2 (One of my favourite series)

When I first started watching anime on DVD, I was a little slow getting into the groove of things, and was unduly harsh to a couple of series. If I were reviewing them today Tenchi Muyo's first two OVA series, the Oh My Goddess OVA and Fruits Basket would be instant tens as well.
