Most embarrasing/worst death ever

Hey, so after sitting and watching fails of the weak on youtube, it's got me wondering; whats the worst death you've had in a video game?

Some of my worst recent ones are - accidentally running my horse off a cliff because i misjudged where the bridge was whilst shooting people behind me on red dead redemption.

lbp2 - replayed the last level in the story mode with a mate on the weekend, when you have to use the grabinator to swing from one flying jumpad to the next before the vaccume eats you, my mate grappled onto me in midair instead of the jumppad and we both fell to our deaths ¬__¬

Major fail - playing through the old donkey kong country, playing through the second mine where you ride the carts... gf said i should go back to another level and get more lives, instead i played on and died.... no autosave on old games = *facepalm* start back from the beginning.

My most recent one was Dead Space. I walked into a room that was glitched so bad that it was just empty black space, so I fell for like 5 minutes, then just died.
 
My biggest fail in recent memory was about a few hours ago when I was playing Golden Axe 2, this lizard dude knocked me very close to an edge and when I got up my character just decided to walk of the bloody thing.
 
During Gears of War co-op, when Aion lured me over to an explosive cannister then blew it up, killing me. But I got him back later.

And in a co-op game of Demon's Souls I died by walking off a cliff, earning me a "D" rating from the person I was (supposed to be) helping. D'oh.
 
First time playing Left 4 Dead Two I took a wrong turning and ended up jumping out of a window only to fall to my death. The other players laughed....
...oh how they laughed! :evil:
 
My attempt at playing Mushihimesama Futari on God mode having never played before was fairly laughable.
A whole continue, with bomb spam, in the first level.
 
after hours of playing smash bros brawl with my best mate, i've learnt that the cannonball attack pikachu does..... is a risky move.

yes it'll fire opponents off the edge, but should i miss... pikachu has no way of getting back into the fight ¬____¬

about 9 times yestardey, *fires self at opponent*
*opponent jumps*
*pikachu plummets to its death*

That attack is more effective for recovery than actual combat. It's the same with Luigi's headbutt.
 
Ah just died gloriously in EVE, trying to solo an incursion.
Could not get enough dps to even kill a frigate.
Only lost 2 days of very lax pve worth of income though.
 
Me and Riva where fighting a R. Ludroth on MH Tri, we where down to 1 more death and we fail and the monster was in its cave wanting to sleep and recover, we attacked it and kept barraging it with hits... then i get caught off guard and accidentally cancelled out my auto-guard = i die and we fail the quest.

(riva died twice so its not all my fault ;) )
 
interesting bot.... had a AI that was able to answer to a thread and actually hold some sense.

OT:

Was going great lastnight, me and a few guys from GameFaq's taking on a rathalos online....ended up carting twice and cost us the quest, gutted when your meant to be partying it up with the elitists and a combo finishes you off in the first life and the second is a OHKO due to my armour being -15% fire resistence.... fighting the king of the skies and fire.

facepalm just doesn't cover it.
