Prior to Scenarist (he switched quite recently... 15 months ago I think) he used DoStudio. The playback judder on "Fist of the North Star" is due to the DoStudio muxer and not PS3/PS4 firmware. A small amount of other discs was released out of specs though.
Scenarist don't have and didn't have any severe issue.

You can have up to 6 episodes/BD50 disc with maxed-out settings and have some small room for short extras (textless opening/ending, spots, promos and similar).
Audio tracks are not played simultaneously (and you even can't have that) but everything is "muxed" in one stream (interleaved). Each M2TS (the bluray muxed files) contains video, all the audio and all subtitles muxed together and *all this* needs to stay in the limits. Doesn't count if you...
Quite unlikely you "re-encode" (meaning: take the JP AVC encoded video and re-encode it again but at a lower bitrate) but almost always you start from the master source (which is a "near lossless" source version) and encode it to your needs. There are still confirmed reports of "re-encode"...
